Level 4 Nuclear Disaster, Idaho, USA
The disaster took place in a experimental power house in 1961. The reaction arose because of a fault of a worker removing a control rod improperly, and the neutrons absorbing in reaction core causing causing a steam explosion and killing 3 of the operators working there. The explosion was very intense, and released 80 curies of Iodine- 131. This was one of the most fatal events to arise in a workplace in the states.
